Welcome to Hollister Pediatrics! Whether you are a returning patient or visiting us for the first time, we are delighted to have you as part of our family. To help you prepare for your visit:
- New Patients: Please download and complete the new patient packet prior to your appointment. If you are transferring from another provider, download and submit a historical medical records request to your previous provider. We will schedule your appointment once we have received your historical records.
- Returning Patients: Please bring your insurance card, update us with any changes in address or phone numbers, and bring your immunization card if you are coming in for a well child check.
